PERMISSION TO ADMINISTER CHILDREN'S BENADRYL(Required)

In the event of an unknown allergic reaction (for example: wasp sting), I give Hand-in-Hand Nature Education permission to administer Children's Benadryl to my child after receiving a verbal confirmation to do so from either a parent/guardian or from 911. The Children's Benadryl will be in its original container, administered by age according to the instructions on the label and dispensed by a Hand-In-Hand Nature Educator. I understand that in the event of a medical emergency due a severe allergic reaction, our Nature Educators will (1) phone the child's parent/guardian and/or 911 and (2) if instructed, administer Benadryl (either orally or topically, as instructed). IN-HOUSE SUBSIDY FUND (optional)

In an effort to make our Early Years Programs as accessible to as many families as possible, we have created our own In-House Subsidy Fund to help low-income families register their children in our Nature Education Programs. Although we follow Island Health’s guidelines in our programs, Hand-In-Hand Nature Education does not qualify for government subsidies as currently, Island Health licenses a program’s premise (not the program itself). As Hand-In-Hand meets outside and does not have a building, we are unable to obtain licensing at this time. The HIH In-House Subsidy Fund works in much the same way the government funds work, except that we raise the funds! (1) I agree as a precondition to my participation in all events organized by “the Company” and/or “the Agents” (collectively referred to as “the Activities”) and in further consideration of “the Company” allowing me to do so, that I will be strictly bound by the terms of this Release of Liability, Waiver of Claims, Assumption of Risk and Indemnity Agreement (“the Agreement”). (2) I acknowledge that “the Activities” involve inherent risks and dangers that may cause serious injury and possible death to participants. (3) I fully understand the risks and dangers associated with my participation in “the Activities” and accept same entirety at my own risk. (4) I hereby waive any and all claims which I may have against “the Company” and “the Agents” and release “the Company” and “the Agents” from all liability for injury, death, property damage or any other loss sustained by me as a result of my participation in “the Activities”, due to any cause whatsoever; including negligence, breach of contract, or breach of any statutory or other duty of care by “the Company” and/or “the Agents”. (5) I appreciate that “the Agreement” limits the liability of “the Agents” to the same extent as it limits the liability of “the Company”, even though “the Agents” are not formal parties to “the Agreement”. I AM THE PARENT/GUARDIAN AND/OR LEGAL GUARDIAN OF THE PARTICIPANT, I HAVE READ AND UNDERSTAND AND AGREE TO EXECUTE “THE AGREEMENT” ON BEHALF OF CHILD/WARD.
